Why insurers
Many Spanish insurers are used to working with clients who already live in Spain, so they may expect a Spanish IBAN, direct debit payment, Spanish ID or NIE, a Spanish address and local bank details. For international students this is often unrealistic — the whole point is that you may need the policy before you are allowed to move to Spain.

No bank / no NIE
Yes, in many cases. Spanish Health Insurance can help students arrange Sanitas cover even where they do not yet have a Spanish bank account, subject to the product and applicant details. This is one of the reasons credit card payment is useful — it can allow students to arrange their policy and certificate while still outside Spain.

Does it affect cert
No. Paying by credit card does not make the policy less suitable for visa purposes. The key issue is not the payment method — it is whether the policy and certificate meet the student visa requirements.
Your certificate should still confirm your full name, policy number, policy dates, medical cover in Spain, no co-payments where required, no deductibles where required, that the insurer operates in Spain, and repatriation wording where required. The reviewing authority is usually interested in the policy and certificate, not whether payment was made by card or direct debit.
Annual vs monthly
Student visa applications often require proof that the insurance covers the full required period. In some cases, paying annually may make the certificate process cleaner because it can show that cover is arranged for the relevant period. However, the correct payment structure depends on the product, insurer process and visa requirements.
Before choosing the payment method, check whether the policy must cover the full academic year, whether the certificate shows annual validity, whether monthly payment is accepted, and whether the consulate expects full-period cover. Choose the option that supports your application properly, not simply the cheapest monthly figure.
Some student policies may be arranged for a fixed study period rather than a standard monthly renewable structure, so the payment and certificate format should be checked before applying.
Which plan
For eligible students aged 14 to 35 coming to Spain for a study stay from 3 to 14 months, Sanitas International Students is usually the first option to check. If the student is over 35, staying longer, applying with dependants or on a non-standard route, Sanitas Residents may be more suitable, subject to current terms and eligibility.
Credit card payment can make either route easier to arrange before arrival. Sanitas International Students may require valid proof of enrolment, matriculation or acceptance from a recognised university, school, language academy or educational institution. Where suitable proof of studies cannot be provided, or where the student does not fit the age, duration or route requirements, Sanitas Residents or another Sanitas option may need to be reviewed. Sanitas Residents may also suit students who are over 35, renewing, applying with dependants or planning to remain in Spain after studying. Payment options should be checked for the specific product and applicant.
From UK/US/Canada
Credit card payment is particularly useful for students applying from outside the EU, including the UK, USA, Canada, Australia, New Zealand, South Africa and other non-EU countries. Many of these students need to arrange insurance before moving to Spain but do not yet have Spanish banking access.
