Profesionales Óptima uses a tiered copay model where a small charge applies to every service from the very first visit. The charge starts at €4 per service and rises gradually depending on how many services you use that year. There is no annual cap on the standard copay — but for most light users, total annual copay costs remain very modest.
Sanitas Profesionales comes in three variants: No Copay (Profesionales), Mid Copay (Profesionales Plus), and Low Copay (this plan). Óptima has the same core coverage as the other two — but does not include dental cover or the death from accident benefit. If budget is the priority and you use the plan infrequently, Óptima delivers the best value.
Profesionales Óptima covers the same core healthcare as the other two variants — with a small copay per service and the same exclusive autónomo benefits.
Total Protection is a support package that activates automatically when you are admitted to hospital for more than 72 hours, or are immobilised at home on medical sick leave for more than 72 hours. These benefits are exclusive to Sanitas Profesionales — they do not exist on standard health plans.
Dental 21 (40+ dental services) is included in the No Copay and Mid Copay Plus variants — but not in this Low Copay plan. If dental cover is important to you, consider upgrading to Profesionales Plus (from €49.60/month), which includes dental with only a €3 copay per service.
Profesionales Óptima uses a tiered annual model. A copay applies to every service from the very first visit, but the per-service charge is kept low — rising only if you use the plan frequently.

* Complex tests / therapeutics: CT scans, MRI, nuclear medicine, endoscopy, chemotherapy, radiotherapy, lithotripsy — these have a fixed €12 copay regardless of how many you use.
** High-frequency services: physiotherapy (10 sessions), ventilation therapy and aerosol therapy (15 sessions), other respiratory therapies — oxygen, BPAP and CPAP (30 days), pre-natal classes. Fixed €20 copay each.
If you use the plan only 5–8 times a year, your total annual copay would be just €28–€52. That makes Óptima ideal for younger, healthier professionals who want full private health cover as a safety net — primarily for surgery, hospitalisation, or unexpected illness — without paying for services they rarely use.
If you're hospitalised or on sick leave for more than 72 hours, Sanitas activates a full support package. These benefits exist because autónomos have no employer to fall back on.
Under Spanish tax law, self-employed workers can deduct health insurance premiums from their IRPF personal income tax. The first €500 per insured person per year — policyholder, spouse and each child — is exempt.
At €37.60/month (€451.20/year), the entire annual premium for Profesionales Óptima falls within the €500 IRPF exemption — meaning the full cost is effectively tax-deductible for most autónomos. Consult your gestor or tax advisor for your specific situation.
All three variants cover the same core healthcare. The differences are in copayment levels and two additional benefits only in No Copay and Mid Copay.

If you have been continuously insured with a Spanish private health insurer (or any Bupa Group company) for 12 or more months, all waiting periods — except childbirth — can be reduced or eliminated on presentation of your previous policy certificate and last payment receipt.
