Application Guide 2026 Sanitas Process Step-by-Step

Health Insurance Application Process SpainHow Sanitas Applications Actually Work

Applying for health insurance in Spain does not need to be complicated. Whether you need private medical cover for a visa, residency application, student stay, EU registration, family relocation or everyday healthcare, the process usually follows a clear sequence: quote, plan recommendation, application details, medical questions, payment, policy issue and certificate delivery.

Spanish Health Insurance helps expats arrange Sanitas health insurance in English, so you understand what information is needed, which plan is suitable, how long the process takes and what happens after your policy is approved.

At a Glance

The Sanitas Application Process at a Glance

Before we go through all 14 steps in detail, here is the whole application process in plain language. This page goes deeper than the broader beginner guide at How to Get Health Insurance in Spain — focusing on the operational mechanics that come AFTER the initial quote.

1You request a quote
2We confirm your situation and insurance needs
3We recommend suitable Sanitas plans
4You choose the policy you want
5We collect the application details
6You answer any required medical questions
7Sanitas reviews the application where needed
8Payment is arranged
9The policy is issued
10You receive your documents and certificate
11Your cover starts on the agreed date
12You can access Mi Sanitas and start using your policy

The exact process depends on the plan, your age, medical history, number of people covered, payment method and whether the policy is needed for a visa or residency application.

Before You Start

What You Need Before Starting

You do not need to have everything finalised before requesting a quote, but it helps to know:

  • Your age or date of birth
  • Your planned province in Spain
  • Your reason for needing cover
  • Your preferred start date
  • Whether the policy is for a visa or residency application
  • Whether you need cover for family members
  • Whether you have any medical history to declare
  • Whether you already have an NIE
  • Whether you have a Spanish or EU bank account

If you are missing some details, you can still request a quote and we will guide you.

1

Request a Health Insurance Quote

The Sanitas application process starts with a quote. Prices in Spain are not one-size-fits-all — they depend on your age, province, plan type, copay choice, medical history and number of people to cover. Without these details, the insurer cannot give an accurate price.

To prepare a personalised quote, we typically need:

  • Your full name
  • Your date of birth (or age band)
  • Your nationality
  • Your planned Spanish province
  • Your town or postcode
  • Number of people to cover
  • Date of birth for each applicant
  • Reason for cover (visa, residency, family, private)
  • Planned cover start date
  • Copay vs no-copay preference (if known)
  • Any known pre-existing conditions to flag
  • Best contact method (email / WhatsApp / phone)
  • Preferred language (we work in English)

Once we have these details we can recommend the most suitable Sanitas plans for your situation — and confirm the exact price before you decide to proceed.

2

Confirm Why You Need Cover

The cheapest plan is not always the right plan. Before recommending a Sanitas option, we confirm the reason for cover — because the requirements vary significantly between use cases.

You may need cover for one of these reasons:

  • Non-Lucrative Visa (NLV) application
  • Digital Nomad Visa (DNV) application
  • Student Visa application
  • EU residency registration
  • Family member or dependant relocation
  • Replacement for or top-up to public healthcare
  • Private cover for faster appointments
  • English-speaking doctor and clinic access
  • Specialist and hospital cover
  • Maternity planning
  • Pre-existing condition management
  • Dental, optical or wellbeing add-on cover

Knowing the reason early helps us match you to a plan that actually meets the requirement — for example a no-copay visa-suitable plan rather than a cheaper copay plan that would be rejected.

3

Choose the Right Sanitas Plan

Sanitas offers a range of plans for different needs. We recommend the option that fits your reason for cover, age, medical history and budget. This page goes deeper than the broader beginner guide — for the broader overview, see How to Get Health Insurance in Spain.

Common Sanitas plan options include:

Sanitas AccedeFrom €22.10/moEntry-level option — lowest cost private coverView plan →
Sanitas AvanzaFrom €33.60/moMid-tier copay plan with broader coverView pricing →
Sanitas Más Salud ÓptimaFrom €34.90/moOptimised cover with copaysView pricing →
Sanitas Más Salud Sin CopayFrom €59.90/moNo-copay private cover — visa-friendly individual planView pricing →
Sanitas International StudentsFrom €42.65/moStudent visa plan for ages 14–35View plan →
Sanitas ResidentsFrom €67.76/moVisa-suitable plan for NLV and DNV applicantsView plan →
Sanitas Residents PlatinumFrom €107.23/moPremium foreign-resident plan with broadest visa coverView plan →

Your exact price depends on age, province, plan type, medical history and number of people. Acceptance and exact terms depend on the insurer's rules.

4

Check Visa or Residency Requirements

If your policy is for a visa or residency application, the plan must meet the requirements of that specific route. Get this wrong and the consulate or immigration office may reject the application.

Non-Lucrative Visa (NLV)

Normally requires a visa-suitable private health insurance with no copays, no waiting periods, full hospital and outpatient cover, and a Spanish authorised insurer. Sanitas Residents is the most common starting point.

Digital Nomad Visa (DNV)

Requirements depend on your social security position. If you are not paying into Spanish Social Security or covered under another EU/Convenio system, a no-copay private health insurance policy is typically required.

Student Visa

Usually requires a visa-suitable student health insurance policy. Sanitas International Students is designed for students aged 14–35 coming to Spain to study and needing cover for use in Spain during their study period. If you are older than 35, contact us because another Sanitas policy may be more suitable.

EU Residency Registration

EU citizens not working in Spain often need to prove private health insurance access to register. A no-copay plan such as Sanitas Más Salud Sin Copay is a common choice.

5

Provide Application Details

Once your plan is chosen, the next step is sending the insurer the personal and contact details required to issue cover. We collect these in English on your behalf.

You may need to provide:

  • Full legal name (as on passport)
  • Date of birth
  • Nationality
  • Passport number and expiry date
  • NIE — if you have one (often not mandatory to start)
  • Spanish province (planned or current)
  • Town and postcode (planned or current)
  • Spanish or correspondence address
  • Email address
  • Phone / WhatsApp
  • Preferred contact language (English)
  • Reason for cover (visa, residency, family, private)
  • Cover start date

Note: an NIE is not always mandatory to start. Many applications can begin with passport details only and the NIE can be added later when you have it.

6

Answer Medical Questions Honestly

Most Sanitas plans require some medical information before cover can be issued. This protects both you and the insurer — and ensures your policy will pay out when needed.

Common medical information requested:

  • Current overall health
  • Existing or chronic conditions
  • Past surgeries
  • Recent hospitalisations
  • Current medication
  • Mental health treatment
  • Pregnancy or planned pregnancy
  • Dental work in progress (for dental cover)
  • Recent diagnostic tests (last 12 months)
  • Family history (in some cases)
  • Allergies and intolerances
  • Lifestyle factors (e.g. smoking)
  • Recent or pending specialist appointments
Always answer medical questions honestly. Non-disclosure can void the policy if you ever need to claim.

Depending on the answers, Sanitas may accept the application normally, request more information, apply specific exclusions, delay cover, or recommend a different plan.

7

Medical Review and Underwriting

If medical information is flagged for review, the insurer's underwriting team will look at the case before issuing cover. This is normal — it doesn't necessarily mean a problem.

Possible outcomes of medical review:

  • Application accepted on standard terms
  • Application accepted with specific exclusions
  • Application accepted with a temporary waiting period
  • Request for further medical information
  • Recommendation of a different plan
  • Application declined in some cases

Underwriting can take a few extra working days. Start the process early if you have a deadline — for example a consulate appointment or visa interview.

8

Choose the Policy Start Date

The cover start date is one of the most important fields on your application — especially for visa applications. The certificate will show this date, and your application can be rejected if the date doesn't fit consulate requirements.

For visa or residency, consider aligning your start date with:

  • Your planned arrival date in Spain
  • Your consulate appointment date
  • The intended visa start / effective date
  • Your university enrolment date (students)
  • Your EU registration appointment date
  • Your TIE / NIE appointment date (after arrival)
  • Your padrón / empadronamiento date (if relevant)

Cover cannot start retroactively. Always choose a start date that gives the insurer time to issue the policy and certificate before any deadline.

9

Arrange Payment

Payment is normally arranged before the policy is issued. The available options depend on the plan and on whether the policy is for a visa or non-visa use.

Common payment options:

  • Spanish bank account direct debit (most flexible)
  • European IBAN direct debit (SEPA)
  • Bank transfer
  • Card payment (some plans)
  • Annual payment (often preferred for visas)
  • Bi-annual / monthly payment (where allowed)

For visa-related policies, annual or bi-annual payment is often preferred or required depending on the product, payment method and application route — even when monthly payment is available on the same plan in non-visa cases.

Visa policies — specific payment options: Annual or bi-annual with European or Spanish IBAN. Annual only with credit card.
10

Policy Issue and Confirmation

Once underwriting is complete and payment is set up, Sanitas issues the policy. You will receive confirmation by email — often the same day, sometimes within a few working days.

When the policy is issued, you should check:

  • Your name is correct and matches your passport
  • Date of birth is correct
  • Other applicants' details are correct
  • Plan name matches what you applied for
  • Cover start date matches what you requested
  • Cover end / renewal date is shown
  • Policy number is on the schedule
  • Premium amount and payment frequency are correct

Errors are easier to correct now than later. If anything looks wrong, flag it before relying on the policy for a visa application or appointment.

11

Receive Your Health Insurance Certificate

For most expat applications, the health insurance certificate is the key document. It is what consulates, immigration offices and universities want to see — not the marketing brochure.

A typical certificate shows:

  • Insured person's full name
  • Insured person's date of birth and nationality
  • Policy number
  • Plan name
  • Cover start date
  • Cover end / renewal date
  • Statement of cover (e.g. no copays, no waiting periods)
  • Insurer's name and authorisation details
  • Issue date and signature / stamp

Keep both digital and printed copies. You may need to upload the digital version online and present a printed copy at an appointment.

12

Receive Your Policy Documents

Beyond the certificate, you will normally receive a set of policy documents describing exactly what is and isn't covered. Skim them as soon as they arrive — they explain what the policy actually does.

Typical policy documents include:

  • Policy schedule (your specific details)
  • Certificate of insurance
  • General conditions (condiciones generales)
  • Special conditions (condiciones particulares)
  • Waiting periods table (where applicable)
  • Exclusions list
  • Authorisation rules (when pre-approval is needed)
  • Emergency contact and procedure
  • Copay table (if applicable)
  • Provider network details
  • Add-on cover details (dental, etc.)

Pay particular attention to waiting periods, exclusions, authorisation rules, emergency procedures, copay amounts and network limits. These are the sections that catch people out later.

13

Set Up Mi Sanitas and Digital Access

Once your policy is active, you can register for Mi Sanitas — the Sanitas member app and online area. It is how you actually use the policy day to day.

Mi Sanitas typically gives you access to:

  • Your digital insurance card
  • Appointment booking (face-to-face and video)
  • Provider and specialist search
  • Hospital and clinic locator
  • Policy and certificate downloads
  • Test result and report access
  • Authorisation requests
  • Blua digital health video consultations (on plans that include it)
  • Member messages and notifications
  • Renewal and policy management

Set this up early. It is much faster than calling for every appointment — and the digital card on your phone is normally enough at clinics and hospitals.

14

Start Using Your Sanitas Policy

Once the start date arrives, you can begin using your cover. Sanitas operates across Spain with a wide network of clinics, hospitals and specialists.

With your active policy you can typically:

  • Visit a Sanitas GP or specialist
  • Book diagnostic tests and scans
  • Use the Sanitas hospital network
  • Access emergency care under your plan's terms
  • Use Mi Sanitas to book and manage appointments
  • Use Blua video consultations (where included)
  • Search English-speaking providers in your region
  • Add dental, optical or wellbeing add-ons later
  • Add or remove family members at renewal
  • Renew the policy annually (continuous cover)

Most expat clients use Sanitas alongside Spain's public system once that becomes available — using private cover for speed and English-speaking access, and the public system as the long-term safety net.

Timing

How Long Does the Sanitas Application Process Take?

It depends on the type of application. Straightforward private cover can sometimes be issued the same day. Visa-related, medically reviewed, family or annual-payment applications usually take a few working days. Allow at least 7–14 days if you have a visa appointment.

Things that can extend timing:

  • Medical underwriting required
  • Pre-existing conditions to assess
  • Multiple applicants on one application
  • Non-Spanish bank account setup
  • Annual payment setup
  • Certificate format requirements
  • Consulate-specific wording
  • Translation or apostille needs (outside scope of insurance itself)
  • Public holidays and weekends

Start the application early — don't wait until the week of your visa appointment.

Before Arrival

Can You Apply for Sanitas Before You Arrive in Spain?

Yes. Many Sanitas applications are started while the applicant is still outside Spain. This is common for visa and student applications, because the policy and certificate are usually needed before the visa is approved or stamped.

Common applicant groups that apply before arrival:

  • Non-Lucrative Visa applicants
  • Digital Nomad Visa applicants
  • Student visa applicants
  • Family reunification applicants
  • EU citizens planning to register on arrival
  • Returning Spanish residents
  • Partners/spouses of Spanish residents
  • Retirees moving from another country

You give the planned Spanish province and postcode at application time. Address details and NIE can usually be updated once you arrive.

No NIE Needed

Can You Apply Without an NIE?

Yes — in many cases. Most Sanitas applications can be started using passport details only. The NIE is typically added to the policy record once you have it.

Banking, payment, certificate format and visa file requirements may eventually require an NIE — but the insurance application itself usually does not need one to begin. We confirm at the time of quote whether your specific plan needs it.

Don't delay your quote waiting for an NIE. Start the application early and add the NIE when it comes through.

No Spanish Bank Yet

Can You Apply Without a Spanish Bank Account?

Sometimes — it depends on the plan. Many Sanitas plans accept a European IBAN (SEPA) for direct debit. Some plans accept card or transfer payment. Fully non-EU accounts can be more difficult, but options often exist.

For visa policies specifically, payment terms can be stricter — annual or bi-annual payment with a European or Spanish IBAN, or annual payment with a credit card. Tell us about your bank situation and we will match the right Sanitas plan to your payment method.

Families

Applying for Family Health Insurance

Each person on a family application is typically a named insured. They each appear on the policy schedule, each have their own certificate where needed, and each have their own application details (especially medical questions).

For each person, we usually need:

  • Full name (as on passport)
  • Date of birth
  • Nationality
  • Relationship to the main applicant
  • Passport number (if available)
  • NIE (if available)
  • Medical history disclosure
  • Whether visa cover is required
  • Cover start date for that person

Don't only cover the main applicant. For visa applications, each adult typically needs their own certificate — and children sometimes need their own certificate too, depending on the consulate.

Pre-existing

Applying with Pre-Existing Conditions

Apply early if you have a pre-existing condition — the insurer may need extra time to review the case before issuing cover. Many applicants with medical history can still be considered, but acceptance always depends on the insurer's underwriting rules. Sanitas may accept the application, request more information, apply exclusions, adjust terms, recommend another plan or decline the application.

Examples of conditions the insurer may ask about:

  • High blood pressure
  • Diabetes (type 1 or type 2)
  • Asthma
  • Heart conditions
  • Cancer history
  • Mental health treatment
  • Joint or back problems
  • Recent surgery
  • Pregnancy
  • Allergies
  • Auto-immune conditions
  • Long-term medication

Possible outcomes include normal acceptance, acceptance with specific exclusions, request for further information, recommendation of a different plan, or decline. Declare honestly and we will guide you through the options.

Visa Applications

Applying for Health Insurance for a Visa

Visa applications are stricter than non-visa applications. The policy needs to meet the route's requirements, and the certificate needs to be in the format the consulate or immigration office accepts.

Common visa application mistakes:

  • Choosing a copay plan when no-copay is required
  • Choosing a plan with waiting periods when none allowed
  • Cover start date that doesn't fit consulate rules
  • Wrong name spelling on the certificate
  • Missing required certificate wording
  • Forgetting dependants
  • Applying too late for the visa appointment
  • Choosing the wrong insurer for the route
  • Annual payment not set up where required
  • Not sending the certificate in time

Tell us the exact visa type when you request a quote — NLV, DNV, student, family reunification, etc. We match the plan and certificate format to the specific route.

Students

Applying for Student Health Insurance

Sanitas International Students is designed for students aged 14–35 coming to Spain to study and needing cover for use in Spain during their study period. It is the first choice for most student applications. If you are older than 35, contact us because another Sanitas policy may be more suitable.

To apply, prepare:

  • Full name (as on passport)
  • Date of birth
  • Nationality
  • Passport details
  • Planned Spanish city and postcode
  • University or institution name
  • Course start and end dates
  • Planned arrival date
  • Any pre-existing conditions to disclose

EU Citizens

Applying for EU Residency Health Insurance

EU citizens often need to show private health insurance to register as residents — especially if they are not working in Spain. No-copay plans such as Más Salud Sin Copay or Más Salud Familias Sin Copay may be suitable depending on whether the application is for an individual, couple or family. The right option depends on age, province, family size and the requirements of the local office.

Things to consider when applying as an EU citizen:

  • Whether you are working in Spain or not
  • Whether you have an S1 or equivalent EU healthcare entitlement
  • Your province (regional rules can differ)
  • Family members who need to register too
  • Planned padrón / registration date
  • Whether you have already started Spanish Social Security
  • Long-term plans for public healthcare access

NLV / DNV

Applying for NLV or DNV Health Insurance

The NLV and DNV are the two most common non-EU residency routes for expat clients — and they have specific health insurance expectations.

Non-Lucrative Visa (NLV)

Requires a visa-suitable no-copay private health insurance policy with no waiting periods, from a Spanish-authorised insurer. Sanitas Residents is the standard option, with Residents Platinum as the premium choice. The certificate format and annual payment setup are both important on this route.

Digital Nomad Visa (DNV)

Requirements depend on your social security position. If you are not paying into Spanish Social Security and cannot evidence an accepted social security arrangement, a private no-copay policy such as Sanitas Residents or Residents Platinum may be appropriate. The correct route depends on your employment structure and application file.

Avoid These

Common Application Mistakes to Avoid

These are the application mistakes we see most often — most of them are easy to avoid once you know what to watch for.

Leaving it too late

Consulate appointments, medical underwriting, and certificate issue all take time. If your visa appointment is in 7 days, requesting cover the day before is risky.

💸

Choosing only by price

The cheapest plan is not always the right plan. For visas, EU registration or families, suitability matters more than the lowest premium.

📋

Not declaring medical history

Hiding pre-existing conditions can void the policy when you need it most. Insurers assess each case individually — declare honestly.

📅

Using the wrong start date

Start date matters for visas, students and EU residency. The certificate often needs to align with your planned arrival or registration date.

👨‍👩‍👧

Forgetting dependants

If you are moving with a partner or children, include them at application time. Each person usually needs their own cover and certificate for visas.

🆔

Assuming an NIE is mandatory

Many Sanitas applications can begin with passport details only. The NIE is typically added later. Do not delay your quote waiting for an NIE.

🎫

Assuming all policies are visa-suitable

Not every Sanitas plan is suitable for a visa application. Visa-suitable plans usually need no copays, no waiting periods and the correct certificate wording.

📜

Not reading the conditions

Waiting periods, exclusions, authorisation rules, copay amounts and network limits matter. Read your condiciones generales before signing.

Common Questions

Application Process — FAQs

The 16 most common questions expats ask about applying for Sanitas health insurance in Spain — process, documents, certificate, timing.

You normally start with a quote. Tell us your age, province, planned start date, the number of people to cover and whether the policy is for a visa, residency, student stay, family or simple private cover. We recommend a suitable Sanitas plan, collect your application details, walk you through any medical questions, arrange payment and issue your policy and certificate. The whole process is handled in English.
Yes. Sanitas is one of Spain's largest private health insurers and many policies are sold to non-Spanish residents — including British, American, EU citizens and other expats. Sanitas Residents, Residents Platinum and Sanitas International Students are specifically designed for foreign residents and visa applicants.
In many cases no — most Sanitas applications can be started with your passport details. The NIE is typically added once you have it. If your application is for a visa, student visa or residency registration, we'll confirm what is needed at the time of quote.
Yes — this is common for NLV, DNV and student visa applicants. You can give your planned Spanish province and postcode, and the address can usually be updated once you arrive. Cover often needs to start on or before the visa date, so plan early.
Typically: your full name, date of birth, nationality, planned Spanish province/postcode, passport details (and NIE if you have it), email, phone, planned start date, number of people to cover, payment details and your reason for cover (visa, EU registration, family, private).
For most Sanitas plans yes. The insurer needs to understand your health history before issuing cover. Questions usually cover existing conditions, recent treatments, medication, surgeries, hospitalisations and lifestyle factors. Answer honestly — non-disclosure can void the policy if you ever need to claim.
Often yes. Pre-existing conditions are reviewed case by case. The insurer may accept normally, request more information, apply specific exclusions, delay cover, suggest a different policy, or in some cases decline. Many applicants with conditions are accepted — declare honestly and we will guide you.
Straightforward applications are often processed within a few working days, sometimes the same day. Visa-related applications, medically reviewed cases, family applications or annual-payment setup can take longer. Don't leave it to the last moment if the certificate is for a visa appointment.
On the agreed start date shown on your policy schedule. For visas, this is usually aligned with your planned arrival or visa effective date. Cover does not start retroactively — you need the policy in place before the start date you ask for.
Yes — within reason. You normally choose a start date when applying, subject to the insurer's rules. For visas, students and EU registration, the date often needs to line up with consulate or appointment requirements.
Once your policy is issued, Sanitas (or we) can provide a certificate in the format your consulate or immigration office expects. Check the name, policy number, dates and cover details before submitting to your visa file. We help with visa-suitable certificate wording in English.
Often yes — depending on the plan. Visa-specific policies may have stricter rules: annual or bi-annual payment with a European or Spanish IBAN, or annual payment only with a credit card. Non-visa private cover is more flexible. We'll confirm the available payment options for your plan.
Sometimes — depending on the plan. A European IBAN (SEPA) is often accepted. A fully non-EU account may not be accepted for direct debit but card or transfer can sometimes work. Tell us your situation and we'll match you to a plan that accepts your payment method.
Yes — Spanish Health Insurance can quote for up to 6 people in one request. You can cover your partner and children under one family-wide policy, or each person on their own. For visas, each adult typically needs their own certificate.
You receive your policy schedule, certificate, policy documents (general and special conditions), digital card, and access to the Mi Sanitas app and member area. Cover starts on the agreed date — you can then book appointments, access providers and use your digital card.
Spanish insurance regulations usually allow a short cancellation/withdrawal period after applying (depending on the policy type and how it was sold). For visa-specific policies, cancellation can affect your residency application. Check the policy's terms or ask us before cancelling.
