New Zealand’s publicly funded health system is built around people who are ordinarily resident in New Zealand. Once you move to Spain permanently, that eligibility does not travel with you. Neither does ACC, which covers accidents that happen in New Zealand rather than injuries sustained overseas. When you move to Spain, your healthcare entitlement is based on your position in Spain rather than the system you used in New Zealand.

Spain has an excellent public health system, but access to it is earned rather than automatic. It generally comes through paying into Spanish social security as an employee or autónomo, through a family member who contributes, or through specific schemes for certain residents. None of those routes is normally in place on the day a New Zealander first applies to move.

Private cover is often what gives new arrivals immediate access to healthcare while their long-term Spanish healthcare position is being established. For most non-EU applicants — and New Zealanders are non-EU — a Spanish private health insurance policy is what demonstrates to the authorities that healthcare is arranged from the moment of arrival. It is also what you actually use while the rest of your residency paperwork catches up.

Over time your position can change. New Zealanders who go on to work in Spain, register as autónomo, or otherwise contribute to the system may become eligible for public healthcare later. These are the practical differences that come up most often in the first year — the things nobody thinks to ask about before they move.

  • Pharmacies do far more. A Spanish farmacia is somewhere you go for advice on minor complaints, not just to collect a prescription. For a lot of everyday problems it is the first stop rather than a clinic.
  • You often go straight to the specialist. Rather than needing a GP referral, private cover in Spain frequently lets you book a specialist directly from the insurer’s directory. Some treatments still need authorisation first, so it is worth knowing which.
  • Private hospitals are not separate insurers. In Spain the private hospitals and clinics are part of a network your policy gives you access to — a distinction that confuses arrivals from systems where the two are more separate.
  • The app changes how you use healthcare. Booking appointments, video consultations and viewing results through Mi Sanitas is normal rather than novel, and it removes a lot of the language friction in the early months.
