Visas
New Zealand citizens moving to Spain often need to consider private health insurance for visa or residency applications, depending on their route and current immigration criteria.
Why it matters
New Zealand’s publicly funded health system is built around people who are ordinarily resident in New Zealand. Once you move to Spain permanently, that eligibility does not travel with you. Neither does ACC, which covers accidents that happen in New Zealand rather than injuries sustained overseas. When you move to Spain, your healthcare entitlement is based on your position in Spain rather than the system you used in New Zealand.
Spain has an excellent public health system, but access to it is earned rather than automatic. It generally comes through paying into Spanish social security as an employee or autónomo, through a family member who contributes, or through specific schemes for certain residents. None of those routes is normally in place on the day a New Zealander first applies to move.
Private cover is often what gives new arrivals immediate access to healthcare while their long-term Spanish healthcare position is being established. For most non-EU applicants — and New Zealanders are non-EU — a Spanish private health insurance policy is what demonstrates to the authorities that healthcare is arranged from the moment of arrival. It is also what you actually use while the rest of your residency paperwork catches up.
Over time your position can change. New Zealanders who go on to work in Spain, register as autónomo, or otherwise contribute to the system may become eligible for public healthcare later. Many still keep private cover alongside it for faster specialist access and English-speaking support, but the reason for holding it shifts from required for the application to chosen for convenience. Requirements vary by consulate and by route, so always confirm what applies to your own case.
Comparison
Most New Zealanders find Spanish healthcare good, and often quicker than expected — but it is organised differently enough to be disorienting in the first few months. These are the structural differences that matter day to day.
| New Zealand | Spain (with private cover) | |
|---|---|---|
| Getting in the door | Enrol with a GP practice; the GP is the gateway | Choose from the insurer’s medical directory, often without enrolling anywhere first |
| Seeing a specialist | Normally via GP referral | Often booked directly through the insurer, though some services need authorisation first |
| Accidents | ACC covers accidental injury in New Zealand | No ACC equivalent — injury care runs through your health cover, subject to policy terms |
| Pharmacies | Mainly dispensing | A first port of call for minor issues and advice, far more involved in everyday care |
| Private care | A supplement to the public system | A mainstream, widely used parallel system with its own hospitals and clinics |
| Digital access | Varies by practice | App-based booking, digital prescriptions and video consultations are standard |
Cover, authorisation rules and available services depend on the plan you hold and where you live. Always check your own policy terms.

Learn Which Plan Fits You →Plan choice
There is no single “New Zealand plan”. The right Sanitas policy depends almost entirely on the route you are taking into Spain, because that determines what the paperwork has to prove.
| Your situation | Usually considered | Why |
|---|---|---|
| Non-Lucrative Visa | Sanitas Residents or Residents Platinum | Designed for non-EU visa and residency applications, with no-copay structures |
| Digital Nomad Visa | Depends on your employment structure | If private cover is your main proof, Residents or Platinum. If you register as autónomo and contribute to Spanish social security, the picture changes |
| Coming to study | Sanitas International Students | Built for student stays; proof of enrolment is normally required to contract it |
| Already resident in Spain | Más Salud options | Once visa-specific wording is no longer the point, broader plans with different copay and price structures open up |
| Older applicants | Reviewed individually | Maximum contracting ages differ between plans, so the options narrow with age and need checking case by case |
Acceptance and exact terms depend on the insurer’s rules and your own circumstances. We check which plans are genuinely open to you before recommending anything.

Why Sanitas
Sanitas is one of Spain’s largest private insurers and has been part of the Bupa group since 1989. For people arriving from abroad, the practical draw is usually the size of the medical network and how much of it works in English.
Network access, benefits and available services depend on the plan you hold and where you live.
The process
Most people arrive here unsure whether they are ready to buy anything. You do not have to be — the first two steps cost nothing and commit you to nothing.
Very fast and responsive to my requirements. Neil was an excellent communicator who guided me through the process with ease.
