Public healthcare in Spain is well regarded and comprehensive. The points above are about the different things private cover adds, not a judgement of the public system.
Two different costs
It helps to separate the two costs. RETA contributions form part of your Social Security position. Public healthcare is available once the corresponding entitlement is active, and eligible family beneficiaries must also be recognised and registered. A private Sanitas premium is a separate, optional cost that buys the added access and choice above; one does not replace the other, and they serve different purposes.
Using both
Many autónomos keep public healthcare for their GP, prescriptions, emergencies and ongoing care — the comprehensive base that comes with their contributions.
They add a private Sanitas plan for private specialist appointments, tests, and the choice of doctor and hospital.
Three set-ups
Some autónomos rely on public healthcare alone — it is comprehensive and comes with your contributions. Private cover is an option, not a requirement.

The Sanitas Profesionales range is built for autónomos, with three ways to pay — Profesionales (no copayment), Profesionales Plus and Profesionales Óptima (copayment options) — all with the full Sanitas network and an autónomo protection benefit for when you cannot work. For qualifying autónomos in estimación directa, eligible health-insurance premiums may be treated as a deductible business expense within the applicable limits — your gestor or tax adviser can confirm your position.
