The value

Private Cover That Also Works for Your Tax Position

As an autónomo you want private healthcare you can rely on. There is a practical bonus: under current Spanish tax rules, eligible private health-insurance premiums can be treated as a deductible business expense in your IRPF. You choose the healthcare cover that suits you, and qualifying premiums may also be treated as a deductible business expense.

How it works

How the Autónomo Health-Insurance Deduction Works

For autónomos calculating their business income under estimación directa, qualifying health-insurance premiums may be treated as a deductible expense up to €500 per eligible person per year, or €1,500 for each eligible person with a disability. This can include the autónomo, their spouse and children under 25 who live with them. Confirm how it applies to your circumstances with your gestor or tax adviser.

Who is coveredAnnual deduction limit
You (the autónomo)Up to €500 per year
Your spouseUp to €500 per year
Each child under 25 living with youUp to €500 per year
A person with a disabilityUp to €1,500 per year

Based on AEAT’s current Renta 2025 guidance, Article 30.2.5ª of the IRPF Law, for autónomos in estimación directa.

Worked examples

What the Deduction Looks Like in Practice

The per-person limit is easier to picture with a few examples. The figures below are the maximum amounts you could deduct — the actual deduction for each person is limited to the eligible premiums actually paid for them.

Who you insureWhat you can deduct
Just yourselfYour eligible premiums, up to €500 for the year
You and your spouseUp to €500 each — up to €1,000 between you
You, your spouse and two children under 25Up to €500 each — up to €2,000 in total
A person with a disabilityUp to €1,500 for that person

If the premium for one person — a child, say — is below €500, the deduction is the premium actually paid, not the full €500. Your gestor applies the correct figures for your household when preparing your return.

Expense, not a refund

A Deductible Expense, Not Money Back

It helps to be clear about what the deduction does. Treating the premium as a deductible business expense reduces the profit your income tax is calculated on — it lowers your taxable income rather than paying the premium back to you. The actual saving depends on your tax rate and your overall position, which your gestor can work out for you.

Keep the paperwork

What to Keep for Your Return

To claim it cleanly, keep the paperwork together: your Sanitas policy documentation, the invoices or receipts for the premiums, and proof of payment such as bank statements. Your gestor uses these to include the eligible premiums in your IRPF return, so having them to hand keeps the process straightforward.

Does it apply to you?

Who Can Claim the Deduction

This deduction is for autónomos who calculate their business income under estimación directa. If you are on a different tax regime, or you are employed rather than self-employed, health insurance is treated differently — your gestor can tell you what applies in your case. The premiums count where they are for you, your spouse or your children under 25 who live with you, and paid by you as the policyholder.

Tax-Deductible Health Insurance FAQs

Under AEAT's current Renta 2025 guidance, the limit is up to 500 euros per year for you, and the same for your spouse and each child under 25 who lives with you. For a person with a disability the limit is up to 1,500 euros. Your tax adviser can confirm how this applies to you.
It can. The premiums for your spouse and your children under 25 who live with you count within the same per-person limits. On the insurance side, they are covered as insured members on your Sanitas plan.
