Who it's for
If you plan to register as self-employed (autónomo) in Spain for the Digital Nomad Visa, your healthcare route can be different — once you register and contribute to Spanish social security you may have public access, with private cover acting as a supplement. Some applicants still need private cover depending on timing, route, documents and family. We check your situation before recommending a policy.
This page is for DNV applicants who plan to work in Spain as self-employed (autónomo) rather than as an employee of a foreign company. If you are a remote employee instead, see DNV cover for foreign employees and the employee vs autónomo comparison. We help with the health-insurance part only; we do not handle the visa, give immigration legal advice, or decide whether you qualify.
The route
If you register as autónomo and pay into Spanish social security, you may gain access to the public healthcare system — which can change what private cover you need for the visa. This depends on your registration, contributions and timing, and on what the authority asks for, so it is not the same for everyone. Confirm the requirements with the consulate, UGE or a qualified immigration specialist. See health insurance for autónomos.
Still useful
Many autónomos keep private cover as a supplement for these reasons even when they have public access.
May still need
Some autónomo applicants still need private health insurance — for example depending on the timing of registration, the exact route, the documents required, family members, or what the consulate or UGE asks for in your case. We do not assume either way: we check your situation first and tell you honestly what is likely needed.
Options
Depending on your case, Sanitas cover can be supplementary private cover alongside the public system, or fuller cover where that is what your application needs. Suitable options include Sanitas Residents, Residents Platinum and other Sanitas plans. We recommend based on your situation; see DNV insurance cost.
Certificate
Where a private certificate is needed for the application, it must show the right cover and dates — see the DNV certificate. We arrange it where your case requires one.
We check first
Because the autónomo route varies, the right answer depends on your specifics. Tell us your plans, timing and family and we will advise honestly whether private cover is needed, useful as a supplement, or not required in your case — before you buy anything.

How we help
Tell us your autónomo plans and we will check what cover (if any) you need and recommend a suitable Sanitas option. We help with the insurance only; visa and social-security decisions rest with the authorities. Get a quote or read the DNV health insurance page.
Important information
Tell us your autónomo plans and we will check what cover you need and recommend a suitable Sanitas option. We help with the health-insurance part of your application. Acceptance and exact policy terms depend on the insurer’s rules; visa decisions rest with the Spanish authorities.
English-speaking Sanitas specialists can help with the health-insurance part of your visa or residency application.
