Get a Quote →Many people arrange private health insurance in Spain at 60 or older, for faster access to specialists and hospitals with English-speaking support. If you are over 60 and applying for a non-EU visa or residence permit such as the NLV or DNV, Sanitas Residents is usually the starting point, with a contracting age up to 75. Sanitas Residents Platinum (contracting age up to 64) may suit some early-60s applicants who want wider premium cover. Sanitas Único is a general 60+ private healthcare option, not the non-EU visa product. This guide explains the options, age limits and what to check, with a personalised quote rather than a fixed price.
Can you
Yes. Being over 60 does not stop you arranging private health insurance in Spain. In fact, Sanitas Único is specifically designed for the 60+ age group and is generally offered without a health questionnaire, which makes it a popular starting point for older applicants. Other plans such as Más Salud are commonly available up to age 75 for new applicants, subject to terms.
Único
Sanitas Único is built for ages 60+, generally with no health questionnaire and no stated upper contracting age in many current cases, subject to current Sanitas terms. That makes it attractive for older applicants who want straightforward access to private care without underwriting hurdles. We confirm what it includes and the current terms for your situation. See Sanitas Único.
Other plans
Depending on your age and needs, other options may suit:
| You want | Sanitas option to consider | Notes |
|---|---|---|
| Straightforward 60+ cover | Sanitas Único | Designed for 60+, generally no questionnaire |
| Comprehensive no-copay cover | Más Salud Sin Copago | Commonly available up to 75 for new applicants |
| Cover on a non-EU visa (NLV) | Residents / Residents Platinum | Visa-suitable; Platinum has a lower age limit |
| EU residency green certificate | No-copay Más Salud | General EU route |
Age
The general picture for a new application (confirm current terms):
| Sanitas plan | Typical age for a NEW application | Notes |
|---|---|---|
| Sanitas Único | Designed for 60+ — no stated upper contracting age in many current cases | Generally no health questionnaire, subject to current terms |
| Sanitas Más Salud / Más Salud Familias | Commonly up to 75 for new applicants | Health declaration applies, subject to current terms |
| Sanitas Residents | Commonly up to 75 for new applicants | For non-EU visa / residency routes, subject to current terms |
| Sanitas Residents Platinum | Maximum contracting age of 64 | Broader / international-style cover — check current terms |
Age limits matter most at the point of application. Once a Sanitas policy has been accepted, contracted and paid, many plans continue without a maximum permanence age — reaching 75, for example, does not automatically cancel a policy you already hold — provided the policy stays active and its terms are met. These figures are a general guide only and should be confirmed against the current Sanitas tariff before buying.
Pre-existing
Many over-60s have some medical history. With Sanitas Único, the general absence of a health questionnaire can simplify things, but other plans assess medical history through the health declaration. Having a condition or taking medication does not automatically mean a refusal. Each case is assessed individually through the health declaration and underwriting, and the outcome can be standard terms, an exclusion, a request for more information, or a decline. Nothing is guaranteed, so a personalised review is essential and you should never cancel existing cover until a new policy is confirmed. See pre-existing conditions health insurance.
Routes
If your move to Spain involves a non-EU visa or residency route (for example the Non-Lucrative Visa or Digital Nomad Visa), the cover usually needs to be visa-suitable, so Sanitas Residents, Residents Platinum or another suitable Sanitas option may need to be reviewed. EU citizens registering residency generally use no-copay Más Salud cover instead. These general pages cover everyday private healthcare; we check the right route for your situation.
Note that Residents Platinum typically has a lower contracting-age limit (64), so older non-EU visa applicants should be age-checked carefully; Sanitas Residents is commonly available up to 75.
Couples
Couples where one or both partners are over 60 can be covered together, each assessed individually — for example one partner on Único and the other on Más Salud. See health insurance for couples.
Cost
There is no single fixed price — it depends on your exact age, the plan, cover level, copay vs no-copay and any medical history. We compare the suitable options, including Sanitas Único, and prepare a personalised quote.

Important information
For third-country applicants over 60 who need health insurance for an NLV, DNV or residence permit, Sanitas Residents is usually the starting point. Its maximum contracting age is 75, so it remains available to most older applicants, and it is a no-copay comprehensive plan with no waiting periods for the visa option (per the policy wording), repatriation included and a certificate once active.
This makes Residents the practical visa/residency choice for the majority of over-60 applicants. Acceptance and the exact terms still depend on your age and the health declaration, so we check your specific case rather than assume — but the age limit itself rarely rules Residents out before 75.
Residents Platinum has a maximum contracting age of 64, so it is available to some applicants in their early 60s but not to older ones. It adds worldwide reimbursement, USA cover, a higher emergency-abroad limit and Dental Milenium on top of the Residents cover, all still no-copay with no waiting periods for the visa option.
If you are in your early 60s and want the wider premium cover — for example because you travel or spend time in the USA — Platinum may be worth considering while you are still within the age limit. Over 64, Residents is generally the one to look at. See the Residents vs Residents Platinum comparison.
EU, EEA and Swiss citizens over 60 do not use the third-country visa route — they register residency (the green certificate / CUE). Where private cover is needed, the usual Sanitas direction is a no-copay Más Salud plan rather than the Residents visa products. See EU residency health insurance.
Permanent residents and people already settled in Spain over 60 may also choose a Más Salud plan for general private cover, or a copay plan, depending on how often they expect to use it. The right option depends on status and how the cover is being used, which we help you work out.
Sanitas Único is a plan designed specifically for older applicants (from age 60), commonly with no health questionnaire, aimed at general private healthcare rather than visa use. It can suit over-60s and retirees who want private cover alongside public healthcare, or who found other plans declined them, subject to the current terms.
Único is not the product for a non-EU visa application — for a visa, over-60s use Sanitas Residents. But for general cover, or as a top-up, Único is worth knowing about. We explain whether Único or a Más Salud plan fits your situation.
At older ages the health declaration matters more, because more people have a medical history to declare. You should declare conditions, medication and recent treatment accurately on the questionnaire. How a condition is treated depends on the underwriting and the policy wording — it may be accepted, accepted subject to terms, excluded, or in some cases declined.
Declaring honestly protects your cover; non-disclosure can affect a claim or the policy's validity. We help you present medical information correctly and explain what is realistic, without promising acceptance or cover for a specific condition. See pre-existing conditions.
Two things mainly determine the outcome for an older applicant: the contracting age limit of the plan (75 for Residents, 64 for Platinum) and the health declaration. Within the age limit, acceptance and terms depend on the individual case. We are honest about this — we do not promise acceptance, and we tell you if a particular plan is unlikely to fit your age or history.
The practical approach is to check the suitable plan for your age first, then work through the medical declaration carefully. For most over-60 visa applicants under 75, Sanitas Residents is both available and suitable, subject to underwriting.
Couples often have an age gap, and sometimes one partner is over a plan's age limit while the other is not. This can mean different plans for each partner, or one plan that accepts both. For a visa application each person needs suitable cover and to be named on the certificate, so we check both ages against the plan limits.
We quote couples together and find the arrangement that covers both — for example Residents for an older partner while a younger partner could consider Platinum — keeping the certificate aligned for the application. See family and couples cover.
Many over-60s moving to Spain are retirees applying for the Non-Lucrative Visa, which is well suited to people not working in Spain. The health-insurance requirement is the same comprehensive no-copay cover, and Sanitas Residents is the usual plan, available up to age 75.
Retirees who split time between countries or travel may prefer the wider international elements of Platinum if they are within the age limit. We match the plan to how you will actually live in retirement. See retiree health insurance.
There is no single price. Premiums rise with age, so over-60 cover costs more than cover for a younger applicant, and the exact figure depends on your age, the plan, your province, the number of people covered, your medical declaration and the current Sanitas tariff. We prepare a personalised quote rather than quote a fixed figure.
Both Residents and Platinum are no-copay, so there are no per-visit charges to add on top; the premium is the cost. See Sanitas pricing for what drives it, and ask us for an exact quote for your age and situation.
Once an over-60 policy is active, you access the full Sanitas medical network in Spain — GPs, specialists, diagnostics and hospital care — with English-speaking support available and the Mi Sanitas app to find providers, including filtering for English-speaking doctors on many plans. On a no-copay plan there are no per-visit charges, which many older residents value when they expect to use healthcare more often.
For visa and residency, the same annual policy stays active through your arrival, the TIE and each renewal — keeping it continuous matters for both your healthcare and your residency status. We help you keep cover in place without a gap as you renew.
We help English-speaking over-60s find the right Sanitas plan for their route and age, check honestly whether a plan is likely to accept the application, guide the medical declaration, and prepare the certificate once the policy is active. We never promise acceptance or a particular outcome — we tell you what is realistic and find the most suitable option.
Tell us your age, your route (NLV, DNV, residency, EU registration or general cover) and any medical history, and we will recommend a suitable plan and prepare a personalised quote. Get a quote or contact an adviser. Final visa and residency decisions rest with the Spanish authorities and depend on your full file.
Over-60s often have more questions than younger applicants, and that is completely normal. Ask us anything about the plans, age limits, the medical declaration, couples with an age gap or timing, and we will explain it clearly in plain English, with no pressure to commit.
Tell us your exact age, nationality, your route (NLV, EU residency or already resident) and any medical history, and we will compare the suitable Sanitas options — including Sanitas Único — and prepare a personalised quote. Please check the actual current policy terms and your personal conditions before purchasing or using any Sanitas policy. Policies change and individual terms can vary.
We check Sanitas Único and the suitable options for your age, in English, with no obligation.
