How
Cover is arranged per person, so a couple can be insured under one family arrangement or as two individual policies. Each partner is assessed individually through the health declaration, so one partner's age or medical history does not necessarily affect the other. We work out the most suitable structure and prepare a combined quote.
Plan vs two
A combined family arrangement can be simpler to manage, while two individual policies can suit couples with very different ages, health or cover needs. Both can use no-copay Más Salud or a copay option. The right choice depends on your circumstances — we compare both.
For example, one partner may prefer a broader no-copay plan while the other may be better suited to a different structure because of age, visa requirements or medical history. Comparing both options can avoid paying for the wrong cover or assuming both partners need the same policy.
Mixed ages
It is common for partners to differ in age or health. The general age picture for a new application (confirm current terms):

Age limits matter most at the point of application. Once a Sanitas policy has been accepted, contracted and paid, many plans continue without a maximum permanence age — reaching 75, for example, does not automatically cancel a policy you already hold — provided the policy stays active and its terms are met. These figures are a general guide only and should be confirmed against the current Sanitas tariff before buying.
Retired couples
Many couples move to Spain to retire. Older partners may suit Sanitas Único (designed for 60+, generally no questionnaire), while younger partners may use Más Salud. See health insurance for retirees and health insurance over 60.
Visa
If your move to Spain involves a non-EU visa or residency route (for example the Non-Lucrative Visa or Digital Nomad Visa), the cover usually needs to be visa-suitable, so Sanitas Residents, Residents Platinum or another suitable Sanitas option may need to be reviewed. Pre-existing
Each partner should declare any medical history, medication or previous treatment where requested. Having a condition or taking medication does not automatically mean a refusal. Each case is assessed individually through the health declaration and underwriting, and the outcome can be standard terms, an exclusion, a request for more information, or a decline. Nothing is guaranteed, so a personalised review is essential and you should never cancel existing cover until a new policy is confirmed.
Cost
There is no single fixed price — it depends on both partners' ages, the plan and cover level, copay vs no-copay, and any medical history. We prepare a personalised quote covering both of you.
