Tax Deductible for Autónomos — Up to €500 IRPF Exempt per Person

Under Spanish tax law, self-employed workers can deduct health insurance premiums from their IRPF personal income tax. The first €500 per insured person per year — policyholder, spouse and each child — is exempt. For a family of three, that means up to €1,500 of annual premium is tax-deductible.

This effectively reduces the real net cost of the plan. Consult your gestor or tax advisor for your specific situation.

⚠ Not a Visa-Approved Plan Sanitas Profesionales is designed for self-employed residents living and working in Spain. Common Questions

FAQ — Sanitas Profesionales No Copay

Questions we get most often about this plan.

Sanitas Profesionales is available to registered self-employed workers (autónomos) in Spain between the ages of 0 and 75. The primary insured must be an autónomo. Their spouse or civil partner and children under 26 years old who live with the policyholder can also be added to the same policy.
All three variants (No Copay, Mid Copay, Low Copay) cover the same core healthcare — primary care, all specialities, CT/MRI, surgery, hospitalisation, Total Protection and accident cover. The differences: No Copay means zero charge per visit or procedure. Mid Copay (Profesionales Plus) charges a moderate amount per consultation. Low Copay (Profesionales Óptima) is the cheapest monthly premium but does not include dental cover or the death from accident benefit. If you use the plan regularly, No Copay often works out less expensive overall.
Total Protection activates when you are admitted to hospital for more than 72 hours, or your GP issues a sick leave certificate and you are immobilised at home for more than 72 hours. Once the 72-hour threshold is reached, you call Sanitas on 91 353 63 48 and they activate the relevant services — home help, childcare, medicine delivery, physiotherapy, taxis and post collection. The premium refund is processed separately. Note: a 2-month waiting period applies to Total Protection from the date your policy first starts.
Yes — a health questionnaire must be completed truthfully by the policyholder and all insured persons. Sanitas uses this to assess the risk and may accept the policy, accept it with exclusions for specific pre-existing conditions, or apply a premium loading. Any known condition not declared at application could result in a claim being denied and the policy being cancelled.
Registered autónomos can deduct health insurance premiums as a business expense under Article 30 of the IRPF law. The first €500 per insured person per year is exempt from IRPF. For yourself, your spouse and two children, that's up to €2,000 deductible annually. You claim this in your annual IRPF tax return (Renta). Keep your Sanitas payment receipts and policy certificate as evidence. Consult your gestor or tax advisor as rules can depend on your tax regime.
Yes. Your spouse or civil partner and children under 26 who live with you can be added to the policy. Each additional insured person receives the full Sanitas network and Total Protection benefits, and each person's premium (up to €500) is independently tax-deductible.
Yes. If you have been continuously insured with a Spanish private health insurer (or any Bupa Group company) for 12 or more months at the time you join Sanitas, all waiting periods — except childbirth — can be reduced or eliminated entirely. You will need to provide your previous policy certificate and your last payment receipt.
No. Sanitas Profesionales is for self-employed residents already living and working in Spain. It does not meet the specific requirements for the Non-Lucrative Visa (NLV) or Digital Nomad Visa (DNV). For visa health insurance you need Sanitas Residents or Sanitas Residents Platinum, which are the only Sanitas plans officially confirmed as meeting NLV and DNV requirements.
