One Health Insurance Policy or Two, for Spain and the USA?

If your life spans Spain and the United States, the practical question is whether a single Spanish policy is enough or whether you also need a separate US arrangement. A Spanish policy is built around care in Spain; Sanitas Residents Platinum adds a dedicated USA Cover benefit plus 90% worldwide reimbursement — each with its own terms and limits — for care outside Spain. Whether one policy is enough, or whether you also want a separate US arrangement, depends mainly on how much time you spend there.

In short

The Short Answer

Policy terms, acceptance, waiting periods and documentation requirements can change, so always check the current Sanitas wording and your personal policy conditions (and any local requirements) before relying on cover or applying.

For many people, one Sanitas Residents Platinum policy can cover life in Spain and give a degree of cover in the United States through its worldwide reimbursement. Whether that is enough depends on how much time you spend in the US and what care you expect there. For short visits it often works; for long stays or routine US treatment, a separate US arrangement may still be needed. A Spanish policy is not designed to replace full US domestic health insurance.

Spanish side

What the Spanish Policy Does

A Spanish visa policy is arranged for healthcare in Spain. Sanitas Residents and Residents Platinum give full cover through the Sanitas network in Spain, with no co-payments and no waiting periods on the visa products. Both also include emergency care abroad for short trips — €12,000 a year on Residents and €30,000 on Platinum, for trips of up to 90 days. That emergency benefit is for urgent problems while travelling, not for planned US care.

USA side

What Residents Platinum Adds for the USA

Residents Platinum includes a worldwide reimbursement modality: you can be treated outside the Sanitas network, including in the United States, pay the bill and claim back 90% of eligible costs up to €500,000 per insured per year, with the sublimits set out in the policy. Alongside that, Platinum carries a dedicated USA Cover benefit for eligible planned treatment in America — a separate benefit with its own scope and authorisation, where Sanitas confirms the treatment before it goes ahead. See how Sanitas medical cover in the USA works → Together, USA Cover and worldwide reimbursement are what makes people say Platinum “covers the USA”. It is genuinely useful, but it is a percentage up to a cap with sublimits — not unlimited or routine US cover, and not a substitute for a US domestic plan if you live there for months.

Sanitas Residents and Sanitas International Students do not include worldwide reimbursement, so US treatment on those plans would fall outside cover except for the emergency-abroad limit on a short trip. If US cover matters, Residents Platinum is the plan with that feature.

When One Policy Works, and When Two Make Sense

Medicare

Where Medicare and US Plans Fit

US Medicare generally does not pay for routine care received in Spain, so it is not your healthcare here — our Medicare and moving to Spain page explains this. Whether to keep US cover is a separate decision to check with Medicare or the Social Security Administration; see keep or drop US health insurance. For the full two-country picture, see the Spain–USA health insurance guide, and Residents Platinum for Americans for the plan itself.

Overlap & gaps

Running Two Policies: Overlap, Gaps and Which Pays

If you do end up with cover on both sides, a little coordination avoids paying twice for the same thing or, worse, leaving a gap. A few practical points:

  • Match each policy to where you are — the Spanish plan for care in Spain, the US arrangement for US care — so they complement rather than duplicate
  • Know which one you would claim on for a given event; worldwide reimbursement on Platinum is a pay-then-claim benefit, not a first-payer network in the US
  • Watch the 90-day trip limit on emergency-abroad cover, so a long US stay does not fall between the two
  • Line up start and renewal dates so neither lapses while you are relying on it
  • Keep invoices and receipts — reimbursement and any US claim both depend on documentation

Two policies are not automatically better; they are better only where each covers something the other does not. For many people one Platinum policy, used sensibly, avoids the cost and admin of a second plan.

Our scope

What We Arrange, and What Stays With a US Provider

To be clear about who does what: we arrange the Spanish private health insurance — the plan, the network access in Spain, the worldwide reimbursement feature on Platinum and the certificate. We do not sell or advise on US domestic health insurance or Medicare, so the US side of a two-policy arrangement stays with a US provider or adviser. What we can do is explain how the Spanish cover behaves for your US time, so you can decide what, if anything, to keep or add on the US side. See the Spain–USA guide, Medicare and moving to Spain and keep or drop US cover.

Important information

Important Information

Important: Sanitas policy benefits, exclusions, waiting periods, authorisation rules, medical network access and visa suitability can change. Cover also depends on the specific policy chosen, the applicant’s personal terms and conditions, health declaration, acceptance terms and any individual exclusions applied by Sanitas. Always check the actual current Sanitas policy wording, certificate wording, general terms and personal policy conditions before relying on any cover or making a visa, medical or financial decision.

FAQs

Spain & USA Cover — Common Questions

Common questions about whether one Spanish policy is enough for Spain and the USA. US cover on Residents Platinum is a percentage up to a cap.

Sanitas Residents Platinum can cover life in Spain and give a degree of US cover through worldwide reimbursement (90% up to €500,000 per year, with sublimits). For long US stays or routine US care, a separate US arrangement may still be needed.
On Residents Platinum, US treatment can be claimed through worldwide reimbursement — 90% of eligible costs up to €500,000 per insured per year, with sublimits. Residents and International Students do not include this, beyond the emergency-abroad limit on a short trip.
No. It is a percentage (90%) up to an annual cap (€500,000 per insured) with sublimits set out in the policy. It is not unlimited or routine US cover.
Not always. For time mainly in Spain with occasional US visits, one Platinum policy often works. For long US stays or routine US treatment, a Spanish policy plus a US arrangement can make more sense.
Generally no. Medicare usually does not pay for routine care received in Spain, so you will need cover in Spain regardless. Keeping Medicare for US time is a separate decision to check with Medicare or the SSA.
Those wanting US and worldwide features usually look at Residents Platinum. Those whose care will be mainly in Spain may find Residents enough. We can compare them and request a quote.
Every visa plan includes emergency care abroad for trips of up to 90 days — €12,000 a year on Residents and Students, €30,000 on Platinum — for urgent problems, not planned care.
Not for living in the US. A Spanish policy is built for care in Spain; Platinum adds reimbursement that helps in the US within a cap, but it is not a substitute for a US domestic plan on long stays.
No. We arrange Spanish private health insurance. For cover while living in the US you would use a US provider; we can explain how the two fit together.
