Spain has one of the best healthcare systems in the world — ranked 7th globally. It runs on two tracks: a universal public system funded by taxes and social security, and a thriving private sector that anyone can access straight away.

For expats, the important thing to understand is timing. Public healthcare is excellent and free, but it only kicks in once you have legal residency and have registered — which can take months. Private health insurance gives you immediate access from day one, lets you choose your own doctor, and is often required by your visa anyway. Most expats start with private insurance and then decide later whether to add public healthcare alongside it.

How Most People Get Set Up

Here is what the process looks like for most of our clients, from first arriving in Spain to long-term settled.

Before you arrive

Take out private health insurance from your home country. This is usually required for your visa application. We issue the policy and visa-compliant certificate remotely — you do not need to be in Spain.

First 6 months

Private insurance covers everything while you wait for residency paperwork. See a doctor the same week, access specialists without referrals, use the Blua app for telemedicine in English.

6-12 months

Residency approved. You can now register for public healthcare at your local centro de salud. Many expats keep their private insurance running alongside it for faster access and English-speaking doctors.

Long term

You choose what works for you. Use public for routine care and private for speed and convenience, keep private only, or transition fully to public. Most of our long-term clients keep private — the speed, choice and English support are worth it.

The questions we get asked most often by expats trying to figure out health insurance in Spain.

It depends on your visa type and situation. If you are applying for a Non-Lucrative Visa, Digital Nomad Visa, Student Visa, Golden Visa or most other residence permits, then yes — private health insurance is a mandatory requirement. Even if it is not legally required, most expats take it out because public healthcare is not available until you have full residency, which can take months. Private insurance fills the gap and gives you immediate access to English-speaking doctors.
Sanitas plans for expats range from around €22 to €150 per month depending on your age, the plan tier and whether you choose copay or no-copay options. For example, a 35-year-old on the Residents plan (no copay, visa-compliant) pays roughly €75-90/month. Students can get covered from €48.60/month. Prices increase with age but are still significantly cheaper than equivalent US or UK private health insurance.
With a copay plan, you pay a smaller monthly premium but a small fee each time you visit a doctor (usually €3-15). With a no-copay plan, the monthly premium is higher but you pay nothing at the point of care — everything is included. For visa applications (NLV, DNV), most consulates require a no-copay plan. If you do not need a visa-compliant certificate, a copay plan can save money if you do not visit the doctor often.
Eventually, yes — once you have legal residency and register at your local centro de salud. This typically takes 3-9 months after arriving in Spain, depending on your visa processing time. Until then, you need private insurance. Many expats keep both running: public for routine care (free), private for speed, specialist access and English-speaking doctors.
