What Worldwide Cover Means for a Retiree in Spain

Policy terms, acceptance, waiting periods and documentation requirements can change, so always check the current Sanitas wording and your personal policy conditions (and any local requirements) before relying on cover or applying.

If you retire to Spain, your day-to-day healthcare is in Spain, so a Spanish plan is the foundation. Sanitas Residents Platinum then adds international benefits: worldwide medical-cost reimbursement — 90% of eligible costs up to €500,000 per insured per year, with sublimits — a separately listed USA Cover, and emergency care abroad of €30,000 a year for trips of up to 90 days. Those are separate benefits with separate terms; the €500,000 reimbursement and the €30,000 emergency limit are not the same pot. Sanitas Residents gives full cover in Spain without the international side, if you do not need it.

The Contracting-Age Point at Retirement Age

This matters at retirement age. Residents Platinum Visado may be taken out until the applicant's 65th birthday; the current IPID states a maximum contracting age of 64. Residents Visado may be taken out until the applicant's 76th birthday. So an older retiree past the Platinum age can still look at Residents, or at Sanitas Único (designed for ages 60+). For American Retirees Specifically

US retirees usually cannot rely on Medicare for routine care in Spain (Original Medicare generally does not cover care outside the United States, with limited exceptions). Platinum's cover for time in the US — its USA Cover and worldwide reimbursement — is governed by the policy's terms, limits and sublimits, and is not a replacement for a US domestic plan if you spend long periods in America. For most American retirees the pattern is a Spanish plan for life in Spain, with the US side considered separately. S1 route

The S1 Route for UK and EU State Pensioners

Not every retiree needs a private plan as their main cover. Eligible UK and EU state pensioners can often register for public healthcare in Spain using an S1 form, issued by their home country's authority, which transfers the cost of their state healthcare to that country. Where an S1 applies, it can satisfy the sickness-insurance side of residence, so private insurance is not strictly needed for that purpose. Many pensioners still take private cover alongside it — for faster access, English-speaking support, or the worldwide and USA features on Platinum — but as a top-up rather than a necessity.

This route is mainly relevant to UK and EU pensioners, not US retirees, who generally cannot rely on Medicare for care in Spain. We arrange the private side; the S1 itself is obtained from your home country's pension or health authority. Tell us if you expect an S1 and we will factor it in.

Health Declaration and Acceptance at Retirement Age

Taking out cover later in life involves a health declaration, and it is fair to be straight about how that works. Acceptance and the exact terms are assessed for the individual, so an existing condition may be accepted, accepted with specific terms, or excluded — it is decided case by case, and no adviser can promise an outcome in advance. Age affects which plan you can take out (Residents Platinum Visado until the 65th birthday; Residents Visado until the 76th; Sanitas Único designed for ages 60+), while your health history affects the terms of whichever plan you take.
