Medicare and Living in Spain: What Actually Applies

Visa rules vary by consulate and change over time, so always confirm the full immigration requirements with the relevant Spanish consulate, an Extranjería office or a qualified immigration specialist.

No cover

Does Medicare Cover Care in Spain?

In general, no. Medicare.gov states that “Medicare usually doesn’t cover health care while you’re traveling outside the U.S.”, with only narrow exceptions that do not apply to living in Spain. Medicare’s own materials note that, in most situations, it will not pay for care or supplies obtained outside the United States, and it does not cover prescriptions bought abroad. The takeaway for Spain is straightforward: Medicare is not your healthcare here.

Part AB

Part A and Part B While Living Abroad

You can generally keep Part A and Part B while living abroad. Part A (hospital) is premium-free for most people, so keeping it typically costs nothing. If you keep Part B, you generally continue paying its monthly premium — even though, per the rule above, it will not pay for your routine care in Spain. Whether to keep or drop Part B while abroad is a US decision with financial and enrolment consequences: check it with Medicare and the SSA rather than treating it as settled.

Advantage

Medicare Advantage (Part C) Abroad

Medicare Advantage plans are built around US provider networks and service areas. Medicare.gov explains that a Medicare Advantage plan can disenroll you if you move outside its service area, in which case you would return to Original Medicare. Living in Spain is outside any US service area, so an Advantage plan does not function as living-in-Spain cover. If you have Advantage, check the consequences of moving with your plan and Medicare.

Medigap

Medigap (Medicare Supplement) Abroad

Some Medigap policies include a foreign travel emergency benefit — on certain standardised plans it helps with emergency care that begins during the first 60 days of a trip, up to limits and a lifetime cap, after a deductible. That is a travel-emergency benefit for trips, not ongoing cover for living in Spain. It does not replace a Spanish plan for your healthcare here.

Keeping or Dropping Part B: a US Decision

A frequent question is whether to keep paying for Part B while living in Spain when it will not cover care here. This is genuinely a US decision, because dropping Part B and re-enrolling later can trigger a lifetime late-enrolment penalty unless a Special Enrolment Period applies, and the rules are individual. We do not advise on it. What we can say is the Spanish half: whatever you decide about Part B, you will still need cover for healthcare in Spain. Check the US-side decision with Medicare and the SSA.

Social Security Benefits While Living in Spain

Separately from Medicare, most US citizens can continue receiving their Social Security retirement benefits while living in Spain — the SSA states that you may receive payments outside the US as long as you are eligible, and there is a US–Spain agreement addressing benefits for people residing in Spain. That is about your income, not your healthcare; your healthcare in Spain still needs Spanish cover. Check your own position with the SSA.

What You Actually Need in Spain

Because Medicare will not be your healthcare here, what you need in Spain is Spanish cover. On arrival that is usually a Spanish private health insurance plan; over time you may become eligible for public healthcare through a route such as an S1, work or another basis, though that is not automatic. Medicare and a Spanish Plan Are Geographic Complements

It helps to think of Medicare and a Spanish plan as geographic complements, not competitors: Medicare (if you keep it) is for care back in the United States; your Spanish plan is for care in Spain. They cover different places, so comparing them like-for-like is misleading. Some people keep Medicare for US trips while holding a Spanish plan for life in Spain; others let Medicare go.
