Most US employer and marketplace health plans are built around US provider networks and generally do not pay for routine care received abroad. Some may offer limited emergency help while travelling, but that is not the same as ongoing cover for living in Spain. In practice, a US plan rarely functions as your healthcare once you are resident here, which is why new arrivals arrange local cover.

US Medicare generally does not pay for healthcare received outside the United States, so it does not cover routine care in Spain.
