The two mean the same thing here: moving from your current private policy to a new one with another insurer. What matters is doing it in the right order: check your renewal date, understand your cancellation deadline, apply for the new policy, wait for acceptance, and then set the start date so there is no break between policies.

Timing
The best time to change is normally before your current policy's renewal date — not after it has renewed, and not after it has expired. Most private policies in Spain renew automatically each year unless you cancel correctly, and cancellation notice is usually required before the renewal date (the exact notice varies by insurer). Start looking two to three months ahead, so there is time to compare Sanitas cover, declare your medical history, obtain acceptance, request any visa or residency certificate, and line up the new start date.
You can usually arrange a Sanitas policy in advance, with the start date set to follow your current cover — so the handover is clean. For example, if your renewal date is 1 September, a Sanitas policy can be arranged in July or August to start on 1 September. Changing mid-year is sometimes possible too, but many annual policies keep charging until the next renewal once they have renewed, so check your terms before you cancel and do not simply stop paying.
No gap in cover
The single most important rule when changing insurer is to keep your cover continuous. Apply for the new Sanitas policy, wait for it to be accepted, confirm the start date — and only then cancel the old one. Never let your current policy expire before the new cover is active. A gap (a lapse in coverage) can leave you without private healthcare, may restart waiting periods, and can show as a break in the healthcare proof used for a visa or residency file.
Waiting periods
Waiting periods are the time before certain benefits — such as hospitalisation, surgery, pregnancy and childbirth, or complex diagnostics — become available on a new policy. When you change insurer, waiting periods may apply again depending on the plan, your previous cover and your medical history. If you have been continuously insured with a Spanish health insurer, or another Bupa Group company, for more than 12 months, standard waiting periods that would otherwise apply may be waived when you provide your previous policy conditions and latest payment receipt. Maternity and childbirth may be treated as an exception. Sanitas still reviews the application, medical history and any exclusions before confirming cover.
Pre-existing
If you have pre-existing conditions, changing insurer needs extra care. You may be asked to complete a medical questionnaire covering diagnosis dates, current medication, recent tests, specialist reports and ongoing treatment. Outcomes vary — a policy may be accepted normally, accepted with exclusions, delayed for more information, or in some cases declined. That is exactly why you should never cancel your existing cover until the new Sanitas policy has been accepted.
Visa / residency
If your private cover is used for a Spanish visa or residency renewal — Non-Lucrative Visa, Digital Nomad Visa, Student Visa, EU registration, family residence, or work routes where private cover is required — the replacement policy must still meet the requirements for your route: comprehensive, valid in Spain, from an insurer authorised here, no-copay where required, supported by a certificate, continuous with your previous policy and covering everyone in the file.
NLV & DNV
You can usually change insurer for the Non-Lucrative Visa, provided the replacement policy stays suitable for the visa or renewal and there is no gap — if your NLV cover ends on 30 June, the new policy should normally start by 1 July. For the Digital Nomad Visa, check which healthcare route applies first: if private insurance is your main proof, the replacement must be full and visa-suitable; if you are covered through Spanish social security, private healthcare may then be used alongside public healthcare.
