Yes — many over-65s take out private health insurance in Spain. The key is that acceptance and terms depend on your age and medical history, assessed through a health declaration and underwriting, so a personalised quote is essential rather than a generic price. Over-65s commonly arrange cover for one of three reasons: a visa or residency requirement (for non-EU applicants), as a private upgrade alongside public healthcare, or simply for faster private access to specialists and hospitals.
Your status matters: a non-EU over-65 applying for a visa needs visa-suitable cover, while an EU citizen or an already-resident over-65 has more general options. We work out which applies to you and prepare an accurate quote.
Which plan
Several Sanitas plans are available to over-65s, with different age rules. This is the quick guide — confirm current terms before buying:

Age limits matter most at the point of application. Once a Sanitas policy has been accepted, contracted and paid, many plans continue without a maximum permanence age — reaching 75, for example, does not automatically cancel a policy you already hold — provided the policy stays active and its terms are met. The figures above are a general guide only and should always be confirmed against the current Sanitas tariff and your own circumstances before buying.
Visa vs non-visa
You will normally need comprehensive no-copay cover suitable for residence — usually Sanitas Residents or Residents Platinum (note Platinum's lower contracting age). See our NLV guide and non-EU retirees page.
You register through the green certificate rather than a visa, so a comprehensive no-copay Más Salud plan is usually the better fit where private cover is needed. See our EU retirees page.
If you simply want faster private healthcare on top of public cover, general plans such as Más Salud or Sanitas Único may suit, depending on age and terms.
Medical history
Most over-65s have some medical history, and that is normal. Blood pressure, cholesterol, type 2 diabetes, a heart history, joint or back issues, a previous cancer, thyroid medication or past surgery all commonly come up.
Having a condition or taking medication does not automatically mean a refusal. Each case is assessed individually through the health declaration and underwriting, and the outcome can be standard terms, an exclusion of a specific condition, a request for more information, or — in some cases — a decline. Nothing is guaranteed, so a personalised review is essential and you should never cancel existing cover until a new policy is confirmed.

Couples
If you are applying as a couple, each person is assessed individually, so one partner may be accepted on standard terms while the other needs more review or an exclusion. Age gaps and different histories are handled per person. This is normal and we simply quote each of you accurately, then put together the right combined cover.
