Health Insurance in Spain for Over-65s

Plenty of over-65s arrange private health insurance in Spain — for a visa, for residency, or simply for faster private healthcare. Acceptance, plan choice and price all depend on your age, your medical history and whether you need visa-suitable cover, so an online estimate rarely tells the full story. This page explains how age and health are handled and which Sanitas plans suit over-65s, then helps you request a personalised quote — with English-speaking help.

Can you

Can You Still Get Health Insurance Over 65 in Spain?

Policy terms, acceptance, waiting periods and documentation requirements can change, so always check the current Sanitas wording and your personal policy conditions (and any local requirements) before relying on cover or applying.

Yes — many over-65s take out private health insurance in Spain. The key is that acceptance and terms depend on your age and medical history, assessed through a health declaration and underwriting, so a personalised quote is essential rather than a generic price. Over-65s commonly arrange cover for one of three reasons: a visa or residency requirement (for non-EU applicants), as a private upgrade alongside public healthcare, or simply for faster private access to specialists and hospitals.

Your status matters: a non-EU over-65 applying for a visa needs visa-suitable cover, while an EU citizen or an already-resident over-65 has more general options. We work out which applies to you and prepare an accurate quote.

Which plan

Which Sanitas Plans Suit Over-65s?

Several Sanitas plans are available to over-65s, with different age rules. This is the quick guide — confirm current terms before buying:

Sanitas planTypical age for a NEW applicationNotes
Sanitas ÚnicoDesigned for 60+ — no upper contracting ageGenerally no health questionnaire, subject to current terms
Sanitas Más Salud / Más Salud FamiliasCommonly up to 75 for new applicantsHealth declaration applies, subject to current terms
Sanitas ResidentsCommonly up to 75 for new applicantsFor non-EU visa / residency routes, subject to current terms
Sanitas Residents PlatinumMaximum contracting age of 64Broader / international-style cover — check current terms

Age limits matter most at the point of application. Once a Sanitas policy has been accepted, contracted and paid, many plans continue without a maximum permanence age — reaching 75, for example, does not automatically cancel a policy you already hold — provided the policy stays active and its terms are met. The figures above are a general guide only and should always be confirmed against the current Sanitas tariff and your own circumstances before buying.

Visa vs non-visa

Visa vs Non-Visa Status for Over-65s

Non-EU, applying for a visa (e.g. NLV)

You will normally need comprehensive no-copay cover suitable for residence — usually Sanitas Residents or Residents Platinum (note Platinum's lower contracting age). See our NLV guide and non-EU retirees page.

EU citizen

You register through the green certificate rather than a visa, so a comprehensive no-copay Más Salud plan is usually the better fit where private cover is needed. See our EU retirees page.

Already resident / want private access

If you simply want faster private healthcare on top of public cover, general plans such as Más Salud or Sanitas Único may suit, depending on age and terms.

Medical history

Medical History and Pre-Existing Conditions at 65+

Most over-65s have some medical history, and that is normal. Blood pressure, cholesterol, type 2 diabetes, a heart history, joint or back issues, a previous cancer, thyroid medication or past surgery all commonly come up.

Having a condition or taking medication does not automatically mean a refusal. Each case is assessed individually through the health declaration and underwriting, and the outcome can be standard terms, an exclusion of a specific condition, a request for more information, or — in some cases — a decline. Nothing is guaranteed, so a personalised review is essential and you should never cancel existing cover until a new policy is confirmed.

Declare everything relevant for the most accurate quote. See our pre-existing conditions guide, health declaration guide and pre-existing conditions quote.

Couples

Over-65 Couples

If you are applying as a couple, each person is assessed individually, so one partner may be accepted on standard terms while the other needs more review or an exclusion. Age gaps and different histories are handled per person. This is normal and we simply quote each of you accurately, then put together the right combined cover.

Mistakes

Common Mistakes Over-65s Make

  • Assuming age alone rules out cover
  • Relying on an online estimate instead of a personalised quote
  • Not declaring medication or conditions
  • Choosing a copay plan where no-copay is needed for a visa
  • Not checking Platinum's lower age limit before setting your heart on it
  • Overlooking Sanitas Único as an older-applicant route
  • Cancelling existing cover before acceptance is confirmed
  • Leaving cover until just before a visa appointment

FAQs

Over-65s — Common Questions

Common questions from over-65s arranging health insurance in Spain. Age and health are assessed individually — always confirm current terms.

Yes — many over-65s take out private health insurance in Spain, for a visa, for residency, or for faster private access. Acceptance and terms depend on your age and medical history through underwriting, so a personalised quote is essential. Sanitas Residents and Más Salud are commonly available up to age 75 for new applicants, Sanitas Único is designed for 60+ with no upper contracting age, and Residents Platinum has a lower limit (64). We check the limits and prepare an accurate quote for your situation.
It depends on your status. A non-EU over-65 applying for a visa usually needs Sanitas Residents or Residents Platinum (note Platinum's lower contracting age). An EU citizen usually fits a no-copay Más Salud plan. An already-resident over-65 wanting private access may consider Más Salud or Sanitas Único. Único is specifically designed for 60+ with no upper contracting age and generally no health questionnaire, subject to terms. We route you by status and age and prepare a personalised quote.
Not a blanket one — it depends on the plan. Sanitas Residents and Más Salud are available for new applicants up to age 75, subject to current terms; Sanitas Único is designed for 60+ with no upper contracting age; and Residents Platinum typically has a lower maximum contracting age (64). So at 65 you usually have options, but the right plan depends on the age rules. We check the current limits and find the suitable route for you.
Yes — medical history and medication are assessed through the health declaration and underwriting, and may affect acceptance, an exclusion, or the terms offered. Routine items like blood pressure or cholesterol medication are common and often manageable; more significant histories may lead to an exclusion, a request for reports, or in some cases a decline. Nothing is guaranteed. Declaring everything relevant gives the most accurate quote, and we handle it carefully without overpromising.
Often, but it depends on the condition and underwriting. Sanitas may accept on standard terms, accept with a specific condition excluded, request more information, or decline. Having a condition does not automatically rule out cover, and many over-65s with managed conditions do arrange policies. It must be declared and reviewed individually. We assess your case carefully and request a personalised quote; we never guarantee cover for a specific pre-existing condition.
It depends on why you need cover. For a non-EU visa such as the NLV, no-copay (sin copago) cover is normally required, so Sanitas Residents or Platinum is the route. For general private healthcare or for EU citizens, a no-copay Más Salud plan is common but a copay plan can sometimes be cheaper and adequate. So no-copay is essential for the visa route and optional otherwise. We help you choose the right structure for your situation and confirm the terms.
Possibly, but check early. Residents Platinum typically has a lower maximum contracting age than the standard Residents plan — commonly 64 — so a new Platinum application at 65 may or may not be available depending on the current terms and your exact age. Sanitas Residents (commonly up to 75) is often the more reliable route at 65+. We check the current contracting-age limits before you set your heart on a particular plan and find the suitable option.
Each person is assessed individually, even on a joint policy, so one partner may be accepted on standard terms while the other needs more review or an exclusion. Age gaps and different medical histories are handled per person. This is normal and does not prevent a combined policy — it just means both partners should disclose their own history so we can quote each accurately. We then put together the right combined cover and a personalised couple's quote.
There is no fixed online price, because age, medical history, plan and cover level all affect it — which is exactly why over-65 cover is quoted personally. An online estimate can be misleading at this age. The honest approach is a personalised quote based on your details. Tell us your age, status and any history through the form and we will compare the suitable Sanitas options and give you an accurate quote, with no obligation.
Use the quote form and tell us your age, status (non-EU visa, EU citizen, or already resident), location and any medical history. We will check the age limits, match the suitable Sanitas plan — Residents, Más Salud or Único — and prepare a personalised quote in English, with no obligation. The more detail you give, the more accurate the result. We never guarantee acceptance, cover for pre-existing conditions, or visa approval.
