Japanese citizens are non-EU for Spanish residence purposes, so you cannot rely on Japanese public or private health insurance once you live in Spain — you need cover that meets the visa requirements and produces the right certificate. Consulate
For the DNV, consulates typically expect comprehensive cover valid in Spain, usually no co-payments, no waiting periods that leave gaps, an insurer authorised in Spain, and a certificate showing the insured person, cover and validity dates. Japanese applicants usually apply through the Spanish Embassy/Consulate in Tokyo — confirm your jurisdiction before applying.
Not enough
Japanese public or private health insurance may be useful at home, but it is not normally accepted as Spanish visa-compliant private cover and does not produce a Spanish residence certificate. Travel insurance is for trips, not residence. Japanese applicants need a comprehensive Spanish residence policy from an insurer authorised in Spain, with the right certificate.
Best options
For the DNV the cover depends on your structure. Japanese employees working remotely for a company outside Spain usually need full visa-compliant private cover — commonly Sanitas Residents Platinum or Sanitas Residents. A self-employed applicant who registers as autónomo and pays Spanish social security may use private cover as a supplement.

Certificate
