Sanitas Health Insurance for Non-EU Families in Spain

When a non-EU family moves to Spain, each member usually needs suitable health insurance for the visa or residency route, and each person is assessed individually. This guide explains how cover works for NLV and DNV families, for children and dependants, and for families with mixed medical history — and which Sanitas plan tends to fit — with English-speaking help throughout.

Do All Non-EU Family Members Need Health Insurance?

Policy terms, acceptance, waiting periods and documentation requirements can change, so always check the current Sanitas wording and your personal policy conditions (and any local requirements) before relying on cover or applying.

Usually yes, for a visa or residency route. Each family member — the main applicant, a spouse or partner, and children — normally needs suitable private health insurance, and each may need to be named on the certificate with the correct start date. Each person is also assessed individually, so one member can be accepted on standard terms while another needs review. We tell you exactly who needs cover for your route and prepare a combined family quote.

NLV families

NLV Families

For Non-Lucrative Visa family applications, the main applicant, spouse or partner and children normally each need suitable private medical insurance. The policy and certificate should show who is covered, the start date, the no-copay structure where required, and the repatriation and residence wording where relevant.

Because the NLV is popular with older applicants, age can matter for the adults — see the age limits below. The usual plans are Sanitas Residents and Residents Platinum. See the full NLV guide.

DNV families

DNV Families

Digital Nomad Visa family cover is more variable. It depends on whether the main applicant relies on private insurance, Spanish social security, or another accepted arrangement. If private insurance is being used as the main healthcare proof, each dependant normally needs to be covered.

Residents Platinum may be the stronger option where private cover is required, while Sanitas Residents may be enough for some cases. Your immigration and social-security advisers should confirm the route; we handle the insurance and prepare the family quote. See the full DNV guide.

Children

Children and Paediatric Access

Children are assessed individually, like adults, and a few practical points matter:

  • Children may need to be named on the insurance documentation, with the correct start date
  • Minors may need parent or guardian details on the application
  • Paediatric access should be checked in your local area
  • School-age children need GP, specialist and emergency access
  • Dental is usually separate and should be checked
  • Each child's medical history must be declared where requested

Check the local Sanitas medical network for paediatric access before choosing.

Different terms

Each Person Can Get Different Terms

This is the key point families miss: a family quote is not simply one price for everyone. Within one family, one member may be accepted on standard terms, another may need a medical review, another may have a specific condition excluded, and a child may need different documentation. One person's medical history does not necessarily affect the others. This is normal and does not prevent a combined family policy — it just means each person is assessed on their own circumstances, which is why we ask each member to disclose their own history.

Residents vs Platinum

Residents vs Residents Platinum for Families

Sanitas Residents

A lower-cost, visa-focused option that suits many NLV and family cases.

Residents Platinum

A broader, more premium option, often stronger for DNV families or families who want more comprehensive benefits.

Age affects the adults' options, so check the limits:

Sanitas planTypical age for a NEW applicationNotes
Sanitas UnicoDesigned for 60+ — no upper contracting ageGenerally no health questionnaire, subject to current terms
Sanitas Mas Salud / Mas Salud FamiliasCommonly up to 75 for new applicantsHealth declaration applies, subject to current terms
Sanitas ResidentsCommonly up to 75 for new applicantsFor non-EU visa / residency routes, subject to current terms
Sanitas Residents PlatinumMaximum contracting age of 64Broader / international-style cover — check current terms

Age limits matter most at the point of application. Once a Sanitas policy has been accepted, contracted and paid, many plans continue without a maximum permanence age — reaching 75, for example, does not automatically cancel a policy you already hold — provided the policy stays active and its terms are met. These figures are a general guide only and should be confirmed against the current Sanitas tariff before buying.

Mistakes

Common Mistakes Non-EU Families Make

  • Assuming only the main applicant needs insurance
  • Assuming children are automatically included
  • Not checking the certificate names and dates
  • Using travel insurance for a residence route
  • Choosing copay cover where no-copay is required
  • Not declaring each person's medical history
  • Leaving insurance until the visa appointment
  • Not checking paediatric access locally
  • Assuming all family members will receive identical terms
  • Not checking whether DNV family members are covered via social security or private insurance

Important information

Important Information

Important: Sanitas policy benefits, exclusions, waiting periods, authorisation rules, medical network access and visa suitability can change. Cover also depends on the specific policy chosen, the applicant’s personal terms and conditions, health declaration, acceptance terms and any individual exclusions applied by Sanitas. Always check the actual current Sanitas policy wording, certificate wording, general terms and personal policy conditions before relying on any cover or making a visa, medical or financial decision.

FAQs

Non-EU Families — Common Questions

Common questions from non-EU families arranging Sanitas health insurance for Spain. Each member is assessed individually — always confirm current terms for your route.

Usually yes, for a visa or residency route. The main applicant, a spouse or partner and children normally each need suitable private health insurance, and each may need to be named on the certificate with the correct start date. Each person is assessed individually, so one member can be accepted on standard terms while another needs review. We tell you exactly who needs cover for your route and prepare a combined family quote.
For a visa or residency route, each covered family member usually needs to be named on the insurance documentation, with the correct start date, so the certificate clearly shows who is covered. This matters because the authorities want to see that every applicant has suitable cover. The exact requirement depends on the route and the product. We make sure the certificate names each member correctly for your application.
Yes, children can usually be included, subject to the current Sanitas terms, age rules and underwriting, and each child is assessed individually. Younger children need paediatric access, so it is worth checking the local network. Where private cover is used for a visa route, children may need to be named in the documentation with the correct start date. Dental is usually separate. We arrange suitable family cover and a combined quote.
For Non-Lucrative Visa family applications, Sanitas Residents and Residents Platinum are the usual options, because they are designed for residence with no copayments and certificate wording for visas. Residents is the lower-cost, visa-focused choice that suits many families; Platinum is broader and more premium. Age can matter for the adults, since Platinum typically has a lower contracting age. We compare both for your family and prepare a combined quote.
It depends on the route. If the main applicant relies on private insurance as their main healthcare proof, each dependant normally needs cover, and Residents Platinum is often the stronger option, with Sanitas Residents enough for some cases. If the family is covered through an accepted social-security arrangement, private cover may be supplementary. Your immigration and social-security advisers confirm the route; we handle the insurance and prepare the family quote.
Each dependant normally needs suitable cover for a visa or residency route, but that does not always mean a separate policy — they can often be included on a family policy where available, with each person named and assessed individually. Whether they are covered under one family policy or arranged separately depends on the Sanitas product, the route and current terms. We work out the most suitable structure for your family and prepare a combined quote.
Yes — this is normal. Each member is assessed individually, so one may be accepted on standard terms while another needs a medical review, an exclusion, or more information, and a child may need different documentation. One person's medical history does not necessarily affect the others. A family quote is not simply one price for everyone. This does not prevent a combined family policy; we assess each person on their own circumstances and prepare an accurate combined quote.
Yes. Each member's medical history is assessed individually through underwriting, so one person's history can affect their own terms — for example an exclusion — without affecting the others. Outcomes depend on the condition and current Sanitas criteria, and nothing is guaranteed. Declaring each relevant member's history honestly gives the most accurate family quote and avoids problems with claims later. We handle each member's history carefully and never promise cover for a specific condition.
For visa and residency routes such as the NLV, no-copay (sin copago) cover is normally required, so Sanitas Residents or Platinum is the route for the family. For families already in public healthcare who simply want private access, a copay plan can sometimes be cheaper and adequate. So it depends on whether the cover is the visa proof or a private upgrade. We help you choose the right structure for your family and confirm it suits your route.
In many cases yes. Families often request a quote and arrange cover before they have NIEs, particularly when applying from outside Spain. Passport details can often be used initially, depending on the product and payment setup, and the details can be updated later once NIEs are issued. Tell us where each family member is in the process and we will work with what you have, then prepare a combined family quote.
For a family visa or residency application, the certificate usually needs to show each covered family member, with the correct start date, so the authority can see that everyone has suitable cover. A certificate naming only the main applicant is generally not enough where the whole family is applying. The exact requirement depends on the route. We make sure the certificate is set up to name each family member correctly for your application.
It depends on the route and what you want. Sanitas Residents is the lower-cost, visa-focused option that suits many NLV and family cases. Residents Platinum is broader and more premium, often stronger for DNV families or families wanting more comprehensive benefits. Age matters for the adults, since Platinum typically has a lower contracting age (64) while Residents is commonly available up to 75. We compare both for your family and prepare a combined quote.
