Get a Quote →If your family already has private health insurance in Spain and you are thinking about changing company, it is important to coordinate the move properly. Each family member must be accepted, the start dates should be aligned, and there should be no gap between your current cover and your new Sanitas policy — especially if your insurance is being used for a Spanish visa, residency renewal, EU residency registration or family residence application.
The short answer
Yes, families can often change health insurance company in Spain — or, in US terms, switch provider. However, family health insurance needs more planning than an individual policy because every insured person must be considered. This may include:
Together
In many cases, yes. Spouses, partners and children can often move to Sanitas together, either on a family plan or under linked policies depending on the product and family structure. When switching together, check:
Dates
When a family changes health insurance provider, mismatched start dates can create problems. The most common one is a parent's cover lining up perfectly while a child's new policy starts a few days late — leaving the child briefly uninsured.
A family no-gap check:
| Family Member | Old Policy Ends | New Policy Starts | Gap? |
|---|---|---|---|
| Parent 1 | 30 June | 1 July | No |
| Parent 2 | 30 June | 1 July | No |
| Child 1 | 30 June | 1 July | No |
| Child 2 | 30 June | 5 July | Yes — needs review |
Cancel first?
Do not cancel your current family health insurance until every family member has been reviewed and accepted under the new Sanitas policy. This matters because:
When
Families should ideally start checking options two to three months before renewal. That gives time to:
Auto-renewal
Many private health insurance policies in Spain renew automatically each year unless cancelled correctly before the renewal date. For family policies this can be more complicated, because there may be one shared renewal date, separate renewal dates, children added later, different start dates for different members, or family members on different products. In many cases, cancellation must be requested at least one month before renewal — but always check the current policy terms.
One has conditions
If one family member has pre-existing medical conditions, the whole switch should be handled carefully. Sanitas may need to review that person's medical history before accepting the policy. This does not necessarily stop the rest of the family applying, but it may affect:
One not accepted
If one family member is not accepted, or is only accepted with exclusions, the family must decide carefully before cancelling existing cover. Possible options may include:
Visa / residency
Family health insurance is especially important when private cover is being used for a Spanish visa or residency file. This may apply to Non-Lucrative Visa family renewals, Digital Nomad Visa family renewals, Student Visa family members, EU residency registration, family reunification, dependent children, and spouse or partner residence applications. Each applicant should be correctly covered, and certificates should clearly show names, policy dates and the type of cover. See change visa health insurance in Spain and family reunification visa health insurance.
NLV
For NLV family renewals, each family member should have suitable private health insurance with no gap in cover. The replacement policy should normally be full private medical cover, no-copay where required, no-deductible, no waiting periods where required and supported by a certificate. If the current family policy ends on 30 June, the new Sanitas policy should normally start no later than 1 July. See change NLV health insurance company.
DNV
For DNV families, the first step is to confirm whether private health insurance is the main healthcare proof or supplementary cover. If private insurance is the main proof, each family member should be covered continuously. If the main applicant has accepted Spanish social security or public healthcare proof, private insurance may be supplementary, but family members should still be checked carefully. See change Digital Nomad Visa health insurance.
EU residency
EU citizens and family members registering as residents in Spain may need private health insurance if they are not covered through employment, self-employment, an S1 / public healthcare or another accepted route. When changing family cover, check whether each family member has valid healthcare proof for EU residency registration. See EU residency health insurance.
What it must include
The right family policy depends on the family's circumstances, but important points include:
Best plans
The right option depends on whether you need general family cover, visa/residency proof, or supplementary cover alongside public healthcare.
Comprehensive no-copay private healthcare with predictable costs — good for families living in Spain, EU residency applicants and households not needing a foreign-resident visa policy. Sanitas Más Salud Familias →
A full private foreign-resident option for visa / residency families needing Spanish documentation at a lower price point — NLV families and some DNV families. Sanitas Residents →
Stronger foreign-resident cover with premium benefits — premium NLV/DNV families, internationally mobile families and those wanting broader long-term support. Sanitas Residents Platinum →
Where public healthcare or social security is the main route and the family wants private cover as an additional layer. copay vs no-copay cover →
Checklist
Run through this before you start — and again before you cancel anything:
| What to Check | Why It Matters |
|---|---|
| Each family member's name | Certificates must be accurate |
| Dates of birth | Affects pricing and policy setup |
| Current policy dates | Needed to avoid gaps |
| Cancellation deadline | Prevents unwanted renewal |
| Medical history | Each person must be reviewed |
| Children included | Do not assume automatic inclusion |
| Visa / residency needs | Policy must match the application |
| Certificate wording | Important for official files |
| Start dates | Should be coordinated |
| No-copay need | Important for some visa routes |
Step by step
Ten steps to move the whole family cleanly, with continuous cover for everyone:
Confirm when each family member's current cover ends or renews.
Find out how much notice is needed.
Include names, dates of birth and relationship.
Confirm whether the policy is needed for an official application.
Declare conditions honestly.
Choose based on family needs, not just price.
Do not cancel existing cover yet.
Coordinate start dates for the whole family.
Make sure documents include everyone required.
Cancel only after acceptance and start-date confirmation.
Mistakes
These are the family switching mistakes we see most often — every one is avoidable:
The ones that cause the most trouble:
| Mistake | Why It Matters |
|---|---|
| Cancelling too early | Family may lose cover |
| Forgetting one person | Visa / residency file may be incomplete |
| Misaligned start dates | Creates gaps in cover |
| Not declaring child medical history | Can affect acceptance |
| Not checking maternity rules | Pregnancy / newborn cover may be limited |
| Waiting too late | Not enough time to coordinate family cover |
| Assuming all family plans are visa-suitable | Requirements vary |
